School Communication Form

There can often be a communication gap between the home and the many education staff members who work with your child. To ensure that each staff member is alerted to either your child’s special needs or those of his or her siblings, a communication form can be developed between you and school personnel. A sample form is provided below for your convenience. This form can be used for a child with cancer or for a sibling of a child with cancer.
